All data which is stored by Greensteps Podiatry, is operated, controlled and processed solely by Lara Cartwright (owner of Greensteps Podiatry and the “Data Controller”).

• Information provided by other healthcare professionals, orthotic and shoe laboratories, and footwear companies.
What type of information we collect
We collect and store any information you provide to us in the form of emails, letters, questionnaires, verbal conversations, forms and assessment notes. This information can include your “personal information”, including your first and last name, address, phone number, e-mail address, medical history, details of your treatment and any other personally identifiable information you provide.
Greensteps Podiatry does not collect and store any “billing information” except insurance details. Any information given to us for billing purposes ie; card details for over the phone payments, is directly inputted into izetl.com (an online card payment service) which does not store card information after use.
Why we collect your information
This information enables Greensteps Podiatry to provide professional podiatry service to patients and to work in partnership with other professionals. Non-identifying information may also be used to review the services provided and to assist Greensteps Podiatry to continue to provide a high level of support and care to her patients. The information that you are asked to provide, and the reasons why you are asked to provide it, will be made clear to you when we ask you to provide it.
Information we collect from third parties
We may also receive personal and non-personal information about you from our colleagues in the NHS and the private sector ie: consultants, physiotherapists, GPs and insurance companies, which will be added to your patient file.
Storage of information
Your information is stored securely in Cliniko (clinical management software) and in gmail. This information may also be accessed by other podiatrists and reception staff, who are contracted to work with Greensteps Podiatry in accordance with the Privacy Policy.
Ongoing communication
Greensteps Podiatry will keep in contact with you via SMS, phone, email or letter. This is an essential part of your assessment and treatment and you will only be contacted for the benefit of your health and wellbeing in relation to Podiatry. Following your initial assessment/s and treatment/s we will contact you periodically, usually by email (by post for patients who prefer or have no email access) as part of the ongoing review process of your health and wellbeing. If we have not received any contact or response from you in three years, your records will then be archived. All electronic information will be deleted after ten years of no contact.
If you are under 18, a minimum annual review appointment is usually required. Greensteps Podiatry will continue to keep in contact with you to arrange these reviews. If we have not had communication from you within three years, your records will then be archived but under 18 records are not deleted until the patient reaches the age of 28 (ten years after reaching age 18/ adulthood) as well as ten years of no contact.
